Practice Info Provide comprehensive primary care to adult patients in an outpatient clinic setting. Sutter Lakeside Hospital is a 25-bed Critical Access Hospital serving Lake County and surrounding rural communities. Only certified trauma and stroke center in Lake County, accredited by The Joint Commission. Offers comprehensive inpatient and outpatient services including emergency care, imaging, ICU, surgery, and specialty clinics. Responsibilities Provide comprehensive primary care to adult patients in an outpatient clinic setting. Perform routine and chronic disease management (e.g., hypertension, diabetes, COPD, heart disease). Conduct physical exams, preventive care, and wellness visits. Diagnose and treat acute medical conditions commonly seen in an ambulatory setting. Order, interpret, and follow up on labs, imaging, and diagnostic studies. Develop individualized care plans based on patient needs and evidence-based guidelines. Manage medication therapy, refills, and adjustments.
