Patient Access Coordinator

To facilitate a smooth patient admission process, the remote Patient Access Coordinator will manage referral activities, oversee admission workflows, and coordinate necessary documentation while collaborating with the Business Development and Central Access and Authorizations Team. Key responsibilities Launch and manage the referral management process, ensuring timely initiation and documentation for patient admissions Coordinate admission logistics, confirming readiness and communicating patient needs to relevant departments Maintain the accuracy of the pending referral log and support the business development team in coordinating patient admissions Required qualifications High School Diploma or GED required; Associates or Bachelor's Degree preferred Preference for a healthcare-related area of concentration or equivalent experience as a licensed healthcare provider Experience in a healthcare-related area is preferred, with post-acute care experience being a plus
