Community Health Navigator — PICHC Outreach

CAMBA INC in Brooklyn, NY seeks a Community Health Worker to interview and evaluate applicants for services, create service plans, and help participants implement those plans. You will also advocate for participants and connect them with needed resources. Responsibilities include outreach to recruit participants, education on childbearing, reproductive health, parenting, and child development, and serving as liaison with referral sources.
