Overview A hospital group in Brookings, OR is currently seeking multiple permanent-hire RN Case Managers to join their growing team. Title Home Health & Hospice Registered Nurse Job Details Job Type: Permanent / Direct Hire Location: Brookings, OR Schedule: Full-time, Monday–Friday, 8:00 am – 5:00 pm (rotating holiday on-call) Pay: $42 – $51.21 per hour Responsibilities Performs professional nursing in the home health program providing direct skilled patient care in the home. Provides home health services that reflect the agency’s philosophy and acceptable standards of care. Provides skilled nursing care including physical assessments, pain management, administration/supervision of medication, catheterization, dressing changes, irrigations, venipuncture, intravenous infusions, central line care, and diabetic care. Completes required documentation (assessments, care plans, clinical notes, agency tracking forms, etc.) in a timely manner in accordance with agency policies. On occasion, may be required to assist in the care of home health patients. Rotates on-call availability as needed to assist and support patient/family through physical and/or emotional crisis situations (24 hr/day). Requirements Must hold a current unencumbered RN nursing license to practice in Oregon. Graduate of an approved school of professional nursing. Current BLWS care or be able to obtain prior to date of hire. One year of nursing practice in a home health, hospice, or facility setting (three years preferred).
