Position Summary The Child & Family Specialist provides direct services, case management, and coordination of care to an assigned caseload of individuals and families. This position assists in accessing needed supports and resources in the community, facilitates ongoing communication and advocacy between the family, DCF, school, probation, and any other involved supports or service providers. This position may also facilitate/co‑facilitate groups in accordance with standard practice. Rate $26.44 Education and Experience / Qualifications BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, Education or related field is required. Previous experience working with client population. Effective time management, strong verbal and written communication skills are required. Essential Duties and Responsibilities Provides data support using the Child First Comprehensive Record to inform program managers and maintain program requirements. Participates in reflective supervision at the individual and team level with Program Manager and Child First teams. Develops case management plans or plans of care and provides direct case management services and/or coordination of care specific to each assigned client and/or family, including interventions to increase treatment completion and improve life functioning. Provides front‑line crisis support via phone, in‑home, or community settings based on assessment. Problem‑solves by interpreting guidelines and consulting with supervisor as needed. Uses judgment to interpret and adapt program guidelines and procedures based on client/family needs. Determines processes and methods based on assessment and strengths, choosing an action plan. Provides group, individual, and/or family counseling with supervision and coordinates treatment phases with internal and external service providers. Maintains records of counseling, coordination, team meetings, and case management for reporting and compliance with Medicaid, licensing, and regulatory requirements. Demonstrates knowledge of psychopathology, crisis theory, and behavioral health interventions relevant to target population. Supports daily program operational functions, transports clients to appointments when appropriate. Provides services in office and community settings, maintaining flexible schedule to meet client needs. Provides safety planning and crisis intervention with supervision, including on‑call coverage for 24‑hour availability. Participates in development, implementation, and facilitation of initial treatment plans, transition plans, and timelines as defined by program. Demonstrates knowledge of referral processes and resources, making timely clinically appropriate referrals. Meets direct service productivity goals, understands program guidelines, goals, and target outcomes, and adheres to practice standards and data protocols. Identifies potential risk factors and implements measures to reduce risk through positive, trauma‑informed, relational interventions. Attends scheduled supervision, team meetings, staff meetings or rounds to coordinate case management and review treatment plans. Seeks additional supervision or consultation as needed and follows supervisory directives. Communicates positively and effectively with clients, families, and providers. Continues to develop knowledge about the history, traditions, values, and cultural aspects of served groups, applying appropriate therapeutic techniques.
