Travel - Temporary Assignment starting 08/25/2026 for 13 weeks with guaranteed 40 hours per week. Weekly pay range: 2,235–2,338 per week. Shift: 5x8 days. Setting: Home healthcare for a travel assignment. This role requires an active Hawaii PT license, relevant home health experience, and demonstrates the ability to work within an interdisciplinary home-based care team.

Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions; participate as a co-leader in interdisciplinary patient group approaches.
- Complete evaluations to ensure quality standards and address assessment areas that require attention.
- Use appropriate clinical techniques and tools; establish individualized treatment plans for patients.
- Provide therapy interventions using approved treatment protocols and techniques.
- Establish individualized care and discharge plans; communicate plans to appropriate parties.
- Complete comprehensive and accurate clinical documentation within established deadlines; supervise assigned assistants; and perform other job-related duties as assigned.
- Document and coordinate care using Homecare Home Base and OASIS as required.

Requirements
- Hawaii Physical Therapy license (active)
- Minimum 2 years of physical therapy experience
- 1 year physical therapy home health experience
- Bachelor's degree required; Master's degree preferred
- Experience with Homecare Home Base and OASIS required
- CPR and BLS (AHA) certifications
- Ability to work in a home health setting and participate in an interdisciplinary care team
