Responsibilities
- Conduct comprehensive PT assessments for patients in home settings and establish an individualized plan of care.
- Implement intervention strategies including therapeutic exercises, gait and transfer training, balance activities, and appropriate manual therapy per scope.
- Monitor progress, adjust the plan of care as needed, and document outcomes in the medical record while ensuring HIPAA compliance.
- Educate patients and family caregivers on exercises, safety, and activities of daily living to support independent function and discharge planning.
- Collaborate with the interdisciplinary team (nurses, physicians, social work, and other therapists) to coordinate care and ensure safe, effective home-based therapy.
- Maintain a safe home environment during therapy sessions and adhere to all infection control practices and clinical guidelines.

Requirements
- Doctor of Physical Therapy (DPT) degree; required
- Physical Therapy licensure in all applicable states of practice; required
- AHA BLS certification; required
- Home health care experience; preferred
- Ability to work in a home-based setting and participate in a travel assignment
