Field-Based LVN Care Coordinator — Hospice & Palliative (Full-Time)
Roze Room Hospice of the Valley
Roze Room Hospice of the Valley is seeking a Clinical Care Coordinator (LVN) for a field-based role serving Culver City and greater Los Angeles. You will conduct nursing assessments, educate families about hospice, and coordinate admissions and transitions to ensure timely and compassionate care. Applicants should hold a California LVN license with 1+ year nursing experience; hospice experience preferred, with strong communication and reliable transportation.
