Practice Setting : Outpatient clinic environment, with seamless integration to hospital-based specialists, diagnostics, and ancillary services for coordinated patient care. Patient Population : Serve a vibrant, diverse community with a mix of preventive care, chronic disease management, acute visits, women's health, and minor procedures. Collaborative Model : Work closely with board-certified physicians, multidisciplinary teams, and support staff in a supportive, team-based atmosphere focused on quality outcomes and patient satisfaction. Schedule : Full-time, Monday-Friday outpatient clinic hours; potential for flexible scheduling; predictable workflow with emphasis on work-life balance. Responsibilities Deliver full-spectrum primary care to patients of all ages Manage chronic conditions, perform preventative care, and address acute health concerns Work closely with physicians and clinical staff to ensure coordinated, high-quality care Maintain accurate and timely documentation in the electronic medical record Qualifications Licensed NP or PA in the state of Illinois Certification as a Nurse Practitioner or Physician Assistant Experience in primary care, family medicine, internal medicine, or urgent care is highly desirable Bilingual skills (Spanish or Polish) a plus, given our diverse patient population Pay Transparency Prime Healthcare Illinois Medical Group offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $130,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
