This Home Health Occupational Therapist opportunity is based in the South County area of San Luis Obispo County, California, serving vibrant coastal and semi-rural communities such as Arroyo Grande, Grover Beach, Pismo Beach, Nipomo and surrounding areas. The pace of life is relaxed and community-oriented, making it an appealing place for clinicians who value balance between meaningful work and an active, outdoor lifestyle.In this role, you will provide one-on-one, patient-centered occupational therapy in the comfort of patients' homes across the South County service area. You will work with a broad adult and geriatric caseload including individuals recovering from surgery or hospitalization, those managing chronic conditions, patients at high risk of falls, and individuals needing support to safely age in place. Your focus will be on maximizing independence with activities of daily living, improving functional mobility, ensuring home safety, and empowering patients and caregivers through education and training.A typical day includes traveling between patient homes within a geographically clustered route, performing initial and follow-up assessments, developing and progressing individualized plans of care, and collaborating with a supportive interdisciplinary team. You will conduct home safety assessments, recommend and train in the use of adaptive equipment, provide activity modification and energy conservation strategies, and design task-specific interventions tailored to each patient's environment and goals. Visit frequency and caseload are structured to allow for thorough, high-quality care and timely documentation, rather than rushed encounters.You will work closely with home health nurses, physical therapists, speech therapists, medical social workers, and home health aides to coordinate comprehensive, goal-directed care. Regular case conferences and team communication are integral to the culture, so you will have ongoing support from clinical leadership and peers as you plan and adjust patient care. This collaborative environment makes it easier to manage complex cases while ensuring patients receive well-coordinated services.The position typically involves day-shift hours with some flexibility to accommodate patient needs and your preferred scheduling patterns within agency guidelines. Patient volume per day is planned to balance productivity with the time needed for high-quality, individualized visits and same-day documentation. Most patients are seen in single-family homes, apartments, or senior living communities, and travel is focused within the South County region to maintain reasonable drive times.This home health role is an excellent opportunity for an Occupational Therapist who wants to build or expand skills in community-based rehabilitation while enjoying an exceptional coastal lifestyle.
