The PTA will deliver care in accordance with physician orders and individualized care plans, under the direction and supervision of a licensed Physical Therapist (PT). This role offers the opportunity to make a meaningful impact by helping patients regain mobility and independence in the comfort of their own homes./ppNow hiring in the following areas:/pulliCentral St. Louis County/liliSouth County/liliNorth County/liliSouth St. Louis City/li/ulpResponsibilities:/pulliUnderstands and adheres to established Agency policies and procedures./liliProvides physician prescribed physical therapy under a plan of care established by the PT./liliImproves or minimizes residual physical disabilities of the patient./liliReturns the individual to optimum and productive level within the patients capabilities./liliParticipates with all other health care personnel in patient care planning./liliPerforms all skilled procedures as ordered by physician and according to the plan of care established by the PT./liliConsults with PT regarding change in treatment./liliInstructs patients and family members in home programs and activities of daily living./liliParticipates in in-service programs and presents in-service programs as assigned./liliParticipates in performance improvement activities as assigned./liliAttends all patient care conferences as scheduled./liliPrepares medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy./liliPerforms services planned, delegated, and supervised by the PT./liliAssists in preparing clinical and progress notes./liliParticipates in educating the patient and family./li/ulpWorks indoors in Agency office and patient homes and travels to/from patient homes/ppSupervised by: Registered Physical Therapist/Director of Therapy, Home Health/ppRisk Exposure: High risk/ppRequirements/ppQualifications:/pp1. Graduate from a two-year college level program approved by the American Physical Therapy Association./pp2. Currently licensed in the state(s) in which practicing./pp3. Two (2) years experience, preferred./pp4. Acceptance of philosophy and goals of Agency./pp5.
