Job Details Location: Bronx, NY 10457 Position Type: Full Time Education Level: Graduate Degree Salary Range: $65,618.00 - $80,000.00 Travel: None Shift: Day Job Category: Nonprofit - Social Services Job Overview Comunliife is looking for experienced Clinical Social Workers to lead their case management team in the OMH Scatter Site program in the Bronx. The Clinical Supervisor is responsible for ensuring that all clinical services meet professional standards as outlined in Comunilife's Employee Handbook and as required by OMH, HPD, and other regulatory agencies. The supervisor serves as manager in charge in the Program Director’s absence and reports to the Program Director. Essential Duties Assign cases to managers and schedule work according to priority. Maintain records and supervision notes; prepare regular and special reports. Oversee quality assurance of case management services to ensure compliance with company and government policies. Perform audits of client charts for comprehensive service planning and accurate charting. Enforce, track, and document home visit policy and procedures. Provide training to new and current case managers. Address dissatisfied clients and handle difficult situations. Receive and report regular supervision with the Program Director. Complete probationary and annual performance reviews for staff. Develop cooperative relationships with ancillary agencies. Prepare and submit all required program statistics and reports on time. Under the Program Director, develop comprehensive group programming in compliance with contract and agency requirements. Coordinate with staff to maintain a minimum monthly occupancy rate of 95%. Coordinate placement of new OMH clients in vacated units within 30 days of referral. Ensure initial and medical assessments are conducted and develop comprehensive service plans with goals. Supervise development of case management service plans and conduct quarterly reassessments. Oversee delivery of mental health and substance abuse services and referrals, including primary medical care and medication adherence. Monitor and update client linkage agreements. Coordinate assistance in obtaining and maintaining government benefits. Ensure case managers secure offsets from clients’ income for state/federal benefits. Assist case managers in locating alternative housing when needed. Complete, sign, and monitor psycho‑social assessments. Conduct weekly supervisory conferences with each staff member. Participate in client interviews and complete intake forms. Qualifications Minimum: Master’s in Social Work or related field; New York State LMSW in Social Work with at least four years of supervisory experience overseeing a case management team. Required experience in individual, family, and group counseling, particularly with HIV/AIDS affected populations and/or mental health and substance abuse individuals who were formerly homeless. Preference for experience in congregate programs and bilingual/cultural proficiency in Spanish.
