THE POSITION

As a Community Health Nurse for the Department of Health, you become part of a team that protects and strengthens community wellness. This role offers daily opportunities to improve lives through responsive public health action. Your work will support important programs across the region. DESCRIPTION OF WORK

This position focuses on assessing community needs and supporting key public health activities that improve the well-being of residents. It also involves direct service, collaboration with partners, and participation in disease prevention and health education activities. As a Community Health Nurse, you will perform the following duties:

  • Data Analysis: Review county level information to identify public health needs and guide program activities
  • Community Collaboration: Coordinate with local partners to support public health initiatives and strengthen community resources
  • Program Development: Create public health initiatives using community data, established protocols, and nursing guidelines
  • Health Monitoring: Track communicable and vaccine-preventable diseases using public health information systems
  • Education Delivery: Provide health education programs to increase public understanding of important health topics
  • Case Support: Offer outreach and case management to promote patient health and safety

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Special Requirements:

  • This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
  • If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

  • You must be able to perform essential job functions.

Legal Requirements:

  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
