Location 929 W Foster Ave,Chicago, IL, 60640-1491,United States Base Pay $75,000.00 - $85,000.00 / Year Employee Type Exempt Full-time Manage Others No Description The Social Worker – IL serves as a key resource for Independent Living residents throughout their experience at The Admiral at the Lake. Responsibilities include coordinating the pre-admission process, conducting psychosocial and cognitive assessments, identifying appropriate services and resources, and helping residents navigate healthcare, financial, legal, mental health, and community resources. This position provides ongoing support to residents and families during changes in health, illness, disability, hospitalization, discharge, and transitions to higher levels of care. The Social Worker coordinates with healthcare providers, facilitates wellness and at-risk resident meetings, supports discharge planning, and helps arrange home care and other community-based services when needed. The role also serves as a liaison between residents, families, internal departments, and outside providers while maintaining appropriate documentation in the electronic medical record (EMR). The Social Worker contributes to campus wellness initiatives, community partnerships, policy development, student supervision, and programs that support the physical, emotional, social, and mental well-being of older adults. Requirements Education & Experience Master of Social Work (MSW) degree required. Minimum of two (2) years of experience working with older adults/aging populations required. Previous experience working in a healthcare, senior living, skilled nursing, hospital, or related setting required. Experience working with individuals with dementia or cognitive impairment required. Licensed Clinical Social Worker (LCSW) preferred. Experience providing psychosocial assessments, care planning, discharge planning, family support, and coordination of community-based resources preferred. Knowledge, Skills & Abilities Strong interpersonal and communication skills with the ability to build trusting relationships with residents, families, staff, and healthcare providers. Demonstrated ability to assess mental, cognitive, emotional, and psychosocial needs and connect individuals with appropriate resources and services. Ability to collaborate effectively with an interdisciplinary team and coordinate services across departments and external providers. Strong organizational and time-management skills with the ability to prioritize multiple responsibilities. Ability to work independently, exercise sound judgment, and proactively identify resident needs. Comfortable facilitating meetings, leading discussions, and speaking to individuals and groups. Strong documentation skills and ability to maintain accurate and timely records in an Electronic Medical Record (EMR). Proficiency with Microsoft Office Suite, including Word, Excel, and PowerPoint. Knowledge of HIPAA, privacy, confidentiality, and applicable healthcare regulations.
