We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the Cranberry Twp area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

\n

\n
• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
\n
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
\n
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
\n
• Maintain accurate, timely, and compliant EMR documentation
\n

• Work independently, but never alone

• Travel to patient homes within assigned territory
\n

LPN Qualifications

\n

\n
• Active Licensed Practical Nurse (LPN) license in the state you work
\n
• Two years of LPN experience and the clinical expertise to deliver quality patient care
\n
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
\n
