Physical Therapist Physical Therapy

Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice. Experience in skilled nursing facility or geriatric rehabilitation preferred. Strong clinical, communication, and organizational skills. Ability to work both independently and as part of an interdisciplinary team.

Evaluate residents to assess mobility, balance, strength, gait, and functional limitations. Develop and implement individualized treatment plans to improve physical function, reduce pain, and prevent disability. Deliver direct therapy interventions, including therapeutic exercises, gait training, and balance retraining. Monitor residents' progress and adjust care plans as necessary to meet therapy goals. Participate in interdisciplinary care team meetings and assist with discharge planning. Educate residents, families, and caregivers on home exercise programs and safety strategies to encourage independence. Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements. Document treatment sessions and progress notes accurately and in a timely manner.
