About the Role

The Community Health Coordinator supports the Food Bank’s efforts to address the intersection of food insecurity and health by assisting with the development, coordination, and implementation of Community Health initiatives. This role works closely with the Community Health Manager, healthcare partners, community agencies, and neighbors to ensure effective program delivery, accurate data collection, and meaningful community engagement across the Food Bank’s 34-county service area.

This position requires a minimum of three in-office days per week and frequent travel by vehicle throughout the service area. In addition to meeting the minimum qualifications, candidates must possess a valid driver's license and have a driving record that meets organizational standards.

Responsibilities:

• Support the planning, coordination, and implementation of Community Health programs, including Food is Medicine initiatives, healthcare partnerships, and wellness interventions.
• Collaborate with healthcare programming to engage new partners and provide ongoing support related to compliance, ordering, and distribution.
• Partner with internal data teams to analyze mapping and chronic disease data to identify community needs and inform Food Is Medicine interventions.
• Maintain program materials, inventory, and supplies to support Community Health initiatives.
• Assist with the development and delivery of community health education, workshops, and presentations.
• Work with internal data teams to collaborate with mapping and chronic disease rate, other Food Is Medicine interventions and needs.
• Maintain accurate inventory records, coordinate inventory acquisitions, and ensure compliance with program reporting and recordkeeping requirements.

Minimum Requirements

Education and Experience:

• Bachelor’s degree in Public Health, Nutrition, Social Sciences, or related field.
• ServSafe Manager’s Certification required or eligible to complete within 90 days of employment.
• 2+ years of experience in program coordination, community engagement, or health advocacy work.
• 2+ years working with diverse populations and communities facing food insecurity or health inequities.
• 2+ years of in-depth understanding of the healthcare system, contractual and regulatory requirements.
• 2+ years of experience in warehouse operations, with the ability to coordinate inventory and collaborate effectively with operational teams.

• Analytical Thinking & Attention to Detail – Adept at gathering, interpreting, and applying data to improve programs and inform decision-making.

WORKING CONDITIONS

• While performing the responsibilities of this position, the employee will work in a variety of settings, including an office environment, warehouse, and off-site partner or community locations. This requires frequent travel by automobile, transit van, or large truck within the Food Bank’s 34-county service area.
• In warehouse and kitchen environments, the employee may occasionally be exposed to moving mechanical parts, vehicles, temperature fluctuations, and moderate noise levels.
