Responsibilities

• Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process
• Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines
• Collect and analyze data, develop and implement a care plan to attain outcomes, and evaluate progress
• Provide patient and family education; participate in discharge planning to ensure continuity of care
• Delegate appropriately and coordinate duties of the healthcare team
• Document care and communication in the patient record, including appropriate use of the EPIC system

Requirements

• Licenses/Certifications: Registered Nurse (RN) license in the state of practice; Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS); Hospital Fire and Life Safety (HLFS)
• Experience: At least two years of recent nursing experience in the field
• Education: Bachelor’s Degree in Nursing (BSN) preferred
• EMR: EPIC experience required within the last 12 months
• Additional: Ability to work in a home healthcare – hospice setting
