CHOICE of NY
CHOICE of NY is seeking a Health & Wellness Specialist to serve as the primary contact for clients, families, and community partners. You will connect clients to healthcare and community resources, coordinate referrals with case managers and providers, and promote access to primary care, insurance assistance, and supportive services.
