Registered Nurse RN Case Manager

Job Summary And Qualifications As a Registered Nurse RN Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care. In this role you will assist our patients in finding the services they need for continuity of care upon leaving the hospital. Our team collaborates closely with our community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME). What you will do in this role You will manage the patients’ care from admission to discharge. You will collaborate with the health care team and develop treatment goals and plan interventions that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, limited finances and/or physical disabilities. You will document patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans. You will provide program orientation to patients/families/caregivers. Orientation will include the case manager's role, Rehab philosophy, and continued stay and discharge criteria. Orientation will also include Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities. You will act as the coordinator of patient/family/caregiver education. You will promote the participation of the patient/family/caregiver in team discussions related to plans, goals, and status. This will be conducted through Family Conferences and other interactions. You will ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences. You will facilitate the involvement of the patient throughout the rehabilitation process. You will document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments. You will communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver. You will assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service. What qualifications you will need Graduate of an accredited school of nursing (BSN preferred). Current Registered Nurse license in Virginia or current Multistate Registered State license. 3+ years of RN experience in a hospital setting preferred. Previous case management or utilization review experience preferred.
