Registered Nurse Case Manager

CVS Health Corporation is seeking an RN Case Manager for field work in Richmond, VA, serving Medicare/Medicaid members with complex needs. You will conduct home, hospital, and community visits, develop care plans, and coordinate services to improve outcomes. Travel up to 50-75% is required, with telephonic and in-person interactions to support transitions of care.
