About the Position

Days | Monday–Friday | 8:30 AM – 5:00 PM | 40 Hours/Week

Responsibilities

• Coordinate discharge planning and case management for medical and emergency department patients.
• Collaborate with physicians, social workers, and the interdisciplinary care team to ensure safe, timely patient transitions.
• Assess patient needs, coordinate post-acute services, and facilitate appropriate utilization of healthcare resources.
• Manage a caseload of approximately 20 patients while maintaining accurate documentation and communication.

Requirements

Requirements

• Minimum 2 years of inpatient hospital Case Management experience with medical-surgical or critical care discharge planning.
• Active Maryland RN license or Compact RN license required.
• BLS certification required.
• Previous Charge Nurse experience required.
• Candidates must be able to work independently with minimal orientation.
