Hours Include - Thursdays. 5-7pm and Saturday. 8-12pm

 

Case Managers at the Family Resource Center provide education, group facilitation, and supportive services to families with children ages 0–5, focusing on early learning, child development, and school readiness. They also provide differential response services for families with children ages 0–5 who are at risk for child abuse and neglect, as well as aftercare services for children ages 0–17 and their families. Case management is a collaborative process involving assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet each individual’s and family’s comprehensive needs through effective communication and connection to resources that promote overall well-being. Services are delivered in Family Resource Centers, client homes, and community settings.

 

This schedule includes weekday shifts, two evening shifts, and a four-hour shift on Saturdays each week.

 

Qualifications:

1. Case Manager I - High school diploma or GED required. Bachelor's degree in Counseling, Social Work or related field preferred.
2. Case Manager II - Bachelor's degree in counseling, social work or related field required. Equivalent experience accepted.
3. Knowledge of community resources preferred.
4. Experience in facilitating group sessions, including parent cafes and educational classes for adults and children preferred.
5. Knowledge of child development framework, including Adverse Childhood experiences (ACEs) and stages questionnaires (ASQ), the Five Protective Factors Survey (PFS) assessment preferred. 
6. Bilingual required.
7. Must be 21 years old or over.
8. A valid California driver’s license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.

Responsibilities:

1. Provide access, advocacy, resources and linkage to clients. 
2. Attend community events, provide outreach services to include identifying areas that need to be strengthened, in addition to setting and reaching targeted goals.
3. Administer program measurement tools and surveys to track and report program data and outcomes.
4. Interact productively within a multi-disciplinary team and other community organizations.
5. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
6. Responsible for preventing and controlling infection.
7. Responsible for maintaining a culture of quality and safety.
8. Effectively use de-escalation skills and techniques for crisis prevention
9. Properly administer the C-SSRS to help prevent suicide.

Other:

1. Attend all required meetings and training.
2. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
3. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
4. Other duties as assigned.
