Registered Nurse RN Case Manager Home Health
Immediate opening for a Registered Nurse RN Case Manager Home Health in Honesdale, PA and surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.
Pay and Benefits
- $38$40 per hour
- $5,000 sign-on bonus
- Weekly pay with direct deposit
- Mileage reimbursement
- PTO and paid holidays
- Health, Dental, AFLAC insurance, Retirement Plan (company match up to 3%)
Schedule
- Monday through Friday 8:30 AM to 5:00 PM
- On-call, weekend, and holiday rotation
Registered Nurse RN Case Manager Responsibilities
- Manage a home health caseload in Honesdale and surrounding areas
- Perform RN assessments and reassessments
- Develop and update care plans
- Coordinate care with physicians and healthcare team
- Complete home health documentation and ensure compliance
- Educate patients and families
Requirements
- Active Pennsylvania Registered Nurse RN license
- Minimum 1 year home health RN experience
- Case Manager or Case Management experience preferred
- CPR certification
- Valid driver's license and reliable transportation
