Program Manager - Care Management

Position SummaryThe Care Management Program Manager position reports to the Director of the IFS Department and assists the Director in developing the goals, resources, and initiatives of the department, ensuring the quality and consistency of our services, the measurement of our impact, and cultivating the well-being and development of our team.  The Program Manager oversees the CM program by supervising a team of care managers and facilitating professional development, providing relevant expertise to various populations and age groups, growth of department, and managing all related aspects.   The Program Manager also provides direct service by maintaining a small caseload of care management clients, working in an integrated team approach providing care management services and supportive clinical counseling to Individual and Family Services (IFS) clients. The Care Management Program Manager identifies needs of clients and provides care management services (resource navigation, benefit application/enrollment, and financial stability improvement). Services are provided at all JFCS locations and will serve the Greater Philadelphia Area, including remote work. The Program Manager will provide services in client homes, office locations, and remote settings; travel throughout the Greater Philadelphia area is required. Duties and Responsibilities Care ManagementCoordinate and provide supportive counseling and care management services for IFS clients through direct services in the client's home, via telehealth, and in the office. Home visits are made to prevent access related issues for the client and made at their request.Conduct comprehensive assessments of clients' needs and assesses their goals to develop mutually agreed upon treatment/care plans. Work with clients to enable access to resources appropriate to their individual needs (e.g., budgeting, financial issues, and accessing public entitlements).Collaborate with community resources (career services, housing) and assists clients in daily life skills to help them function at as high a level as possible.Maintain knowledge of a broad variety of community resources and refer clients when appropriate to support their treatment goals.Provide advocacy with landlords, community enforcement agencies, family and natural supports, and others as needed. Prepare and document accurate and timely records including progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies.Participate in team conferences, staff trainings, and other meetings related to care management issues. Leadership and Administrative Responsibilities  Provide an open and engaging environment where employees can communicate freely and work together to improve service to our clients. Serve as a resource to the IFS team. Lead the team by providing open communication, ongoing coaching and feedback and ensuring performance objectives are met. Recognizes good performance and addresses areas for improvement with individual team members. Lead the team by means of regularly scheduled individual supervisions and case reviews with respect to assessment skills, use of resources and documentation. Lead and monitor the team's work functions, productivity, time management, caseload management and crisis management. Ensure team members document accurate and timely records including progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies.Facilitate team meetings/group supervision to enable opportunities for the team to share issues and discuss programs and to ensure that team has adequate communication of agency news and policies. Provide stretch opportunities so that employees learn and grow in scope, responsibility, and expertise. Ensure new employee selection is handled according to level of expertise required, capability and team "fit." Ensure new employees are oriented and trained to perform their professional function consistent with program policies and agency's standard of performance. Create meaningful team evaluations so that associates can understand their strengths and where they need improvement.  Meet regularly with the Director to plan and coordinate activities and assess staff performance. Inform the Director of any developing service needs, team issues, or administrative concerns and provide possible resolution options/recommendations and plans. Attend appropriate administrative, leadership team and Board Advisory committee meetings both within and outside of JFCS, as assigned.   Recommend initiatives to enhance the overall activities consistent with the agency strategic plan. Participate actively in strategic discussions when appropriate, considering Department scope of service as well as program planning, marketing, implementation, and development issues.  Work with Director to develop new community collaborations and partnerships, help to maintain existing community relationships, both formal and informal, and represents IFS and Agency with community organizations and professional groups. Monitor program specific expenses. Ensure accurate timekeeping is kept and submitted on a timely basis. Perform other duties as assigned by immediate leader and other management as requested.  CompetenciesMotivated individual to engage clients and help tackle financial challenges and achieve goals.Ability to multi-task, problem solve and think outside the box.Knowledge of emerging and innovative techniques Ability to work effectively and compassionately with individuals of various ethnic and religious groups and to understand the meaning of cultural competency Able communicator, who can rapidly assess his/her audience and adjust message to best suit each listener; sound listening skills; ability to inspire and mobilize his/her team to move in a unified direction.Independent worker - can follow up and follow through.Demonstrate a professional demeanor no matter what the situation; must be able to meet and interact professionally, patiently and courteously with a diverse population of clients and families.Must be able to exercise independent judgment in an area for which procedures cannot be standardized.Must be able to maintain confidentiality.Ability to multi-task priorities essential to program Solve problems creatively and resourcefully Capacity for working with team at all levels as well as with board and lay community.Flexibility and the ability to adapt to changes in schedule and assignments. Demonstrates initiative in contributing to the development of the department's programs and services as well as his/her own professional development. Must be in good standing, maintain any pertinent license(s), and follow guidelines as required by the PA State Board of Social Workers, Marriage and Family Counselors, and Professional Counselors. Demonstrate knowledge, understanding, and implementation of practices based on the NASW code of ethics. Participate in annual Trauma Informed Care training(s). RequirementsMaster's degree from an accredited graduate school (MSW preferred)2-4 years professional experience in a related field (social services, counseling, case management) and 2 years supervisory experience (may be concurrent).Extensive background, knowledge and skill in counseling and/or care management.Proficient use of technology (smart phone/tablet/laptop) and experience using MS Office applications is required. Additional RequirementsThis position may be required to spend some time in all our current and future Greater Philadelphia Area offices each week, virtual work will also be required.  Travel is a must.Some evening or weekend hours may be required based on program need.Employee must have access to a car.Employee must have a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive.
