Licenses: RN(Compact Accepted)
Certifications: ACLS-AHA; PALS-AHA; CERT-TNCC; CERT-NIH Stroke Scale; CERT-Crisis Prevention Intervention
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year

Level 1 or 2 Trauma center experience
CPI Blue Card must be obtained within 30 days of start if RN is not yet certified
