Registered Nurse - Case Management

Schedule

This is a full-time day position with 3-12 hour shifts per week, including weekend rotation and no call required.

Job Description

The Registered Nurse will assist patients and their families in managing their care plans across the continuum for both inpatients and outpatients. This role requires collaboration and effective communication to navigate complex healthcare needs.

Responsibilities

• Manage the overall care plans for patients across the healthcare continuum.
• Provide support to patients and their families in navigating the healthcare system.
• Document and maintain patient records in accordance with hospital policies.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
• State licensure as a Registered Nurse (RN).
• A minimum of 3 years of RN Case Management experience is required.
• EPIC experience is mandatory.
• Flu vaccine required - no exemptions.
