Registered Nurse – Case Management

Schedule: 3-12 hour shifts per week, full-time day position with weekend rotation. No call.

Assignment Details:

• Contract Length: 4 weeks
• Guaranteed Hours: 36 hours per week

Requirements:

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
• State licensure as a Registered Nurse (RN)
• Minimum of 3 years RN Case Management experience
• EPIC experience required
• Flu vaccine required; no exemptions

Roles & Responsibilities:

• Assist inpatients and outpatients and their families in managing their plan of care across the continuum.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Evaluate patient care plans and make necessary adjustments based on patient needs and best practices.
• Educate patients and their families about available resources and options for care.
