RN Clinical Coordinator, Heart and Vascular

Lead programmatic development for assigned Heart and Vascular programs through care coordination and outreach management Serve as the primary clinical-expertise contact and participate in care-pathway development Cultivate, implement, and monitor evidence-based standards of care Coordinate interdisciplinary care and post-discharge education for patients and families Assess patients, collect and analyze clinical data, identify nursing diagnoses, and document interventions and care plans Facilitate communication among referring hospitals, providers, EMS, patients, departments, and OhioHealth service lines Provide community education and assigned training Ensure readiness and compliance with regulatory, accreditation, licensing, and Joint Commission requirements Implement strategic initiatives, improve program value, participate in process improvement, and support cost-of-care initiatives Participate in program-advancement forums and maintain knowledge of best practices and emerging trends Provide clinical care as needed, both in person and virtually Requirements Bachelor's Degree required Nursing or a related healthcare field, completed or in progress Current Ohio licensure as a Registered Nurse Experience in clinical program development and operations Minimum of three years of experience in assigned program(s) Ability to work in a complex clinical and/or administrative system or program Communication skills to lead, teach, and influence others High level of analytical ability Core Competencies Demonstrates expertise in clinical program development and operations, with a focus on care coordination, evidence-based standards, and regulatory compliance. Proficient in patient assessment, data analysis, and interdisciplinary communication to enhance program value and improve patient outcomes.
