Job Title

LPN – Skilled Nursing / Long-Term Care

Location

Brookings, SD

Schedule / Job Type

Contract | Nights (6:00 PM – 6:30 AM) | Onsite

Key Responsibilities

• Provide direct nursing care to residents across long-term care, rehabilitation, and memory care units
• Administer medications using bubble pack distribution system; no IV medications required
• Perform clinical documentation using Point Click Care electronic charting system via hallway kiosks
• Float across multiple hallways and neighborhoods as needed based on census, acuity, and staffing needs
• Collaborate with CNAs, Med Aides, and other care team members during night shift operations
• Participate in a 2-hour facility orientation prior to first shift, including building tour, charting overview, and resident introductions

Required Qualifications

• Minimum of 2 years of experience as an LPN in a Long-Term Care or Skilled Nursing Facility setting
• Active LPN license to practice in the State of South Dakota
• Current BLS certification issued through the American Heart Association
• Proficiency or ability to learn Point Click Care electronic health records system

Preferred Experience / Skills

• Experience working in memory care or rehabilitation unit environments
• Familiarity with consistent staffing models in long-term care settings
