Your Role You will manage the ongoing medical needs of residents in Eugene-area skilled nursing and long-term care facilities, addressing acute concerns and chronic conditions. Working closely with physicians and interdisciplinary facility teams, you will deliver individualized care and maintain a consistent, responsive clinical presence. Key Responsibilities Perform comprehensive histories, physical examinations, admission assessments, and follow-up visits. Evaluate, diagnose, and manage acute and chronic medical conditions. Develop individualized treatment plans and prescribe medications and therapies within your scope of practice. Order, review, and interpret laboratory tests, imaging, and other diagnostic studies; address results promptly. Provide post-hospital follow-up and transitional care. Assess and respond to changes in residents’ cognitive, behavioral, functional, and medical status. Educate residents, families, and caregivers about diagnoses, treatment options, and care plans. Participate in interdisciplinary care planning with nursing, therapy, dietary, social services, and administrative teams. Coordinate care with your collaborating physician and other specialists. Support efforts to reduce avoidable emergency department visits and hospital readmissions. Maintain accurate, timely electronic medical record documentation and comply with applicable CMS, state, and facility requirements. Qualifications Required Graduation from an accredited Nurse Practitioner or Physician Assistant program Active, unrestricted Oregon license, or ability to obtain one National board certification or board eligibility by the start date DEA registration and prescriptive authority, or ability to obtain them National Provider Identifier (NPI) Eligibility to participate in Medicare and Medicaid Current Basic Life Support (BLS) certification Preferred Experience in skilled nursing, long-term care, geriatrics, internal medicine, family medicine, or post-acute care Experience managing medically complex patients and chronic conditions Familiarity with CMS documentation and regulatory requirements Successful candidates communicate clearly, work independently with sound clinical judgment, and value collaboration with residents, families, and care teams.
