You will assess needs, develop comprehensive plans, coordinate with nurses, and conduct regular in-person and phone check-ins with frail elders and adults with disabilities. The role requires a Bachelor's degree in human services or related field with 1+ year of relevant experience or 3+ years in another field, plus a valid driver's license and insurance.
