**This is a posting for a position with the Care Transformation Collaborative-RI**Program Manager, Rural Health Transformation-Care Transformation Collaborative-RIFULL-TIME-HYBRID 2 to 4 days per month in office About UsThe Care Transformation Collaborative of Rhode Island (CTC-RI) is Rhode Island's statewide primary care transformation organization. We bring together healthcare providers, health plans, state agencies, employers, academic institutions, and community organizations to strengthen primary care, improve healthcare access, advance integrated behavioral health, support workforce development, and drive healthcare transformation across the state.Position SummaryCTC-RI is seeking a Program Manager to support implementation of Rhode Island's Rural Health Transformation Program.This role leads project planning, provider engagement, stakeholder coordination, reporting activities, and implementation support for initiatives designed to improve healthcare access, strengthen primary care, and advance healthcare transformation in rural communities.Key ResponsibilitiesLead implementation of assigned program initiativesDevelop project plans and implementation timelinesCoordinate provider participation and stakeholder engagementFacilitate meetings, workgroups, and collaborative activitiesMonitor program performance and reporting requirementsSupport technical assistance and quality improvement effortsCoordinate contractors, consultants, and external partnersQualificationsRequired:Bachelor 's degreeFive years of healthcare project or program management experienceStrong project management and communication skillsPreferred:Public health, healthcare transformation, or quality improvement experienceExperience working with healthcare providers or healthcare systemsWhy Join CTC-RI?Help lead one of Rhode Island's most significant healthcare transformation initiatives and support improvements in healthcare access and outcomes for rural communities. We are an Equal Opportunity Employer.
