Remote LTSS Care Manager — Holistic Long-Term Care Planning

Centene Corp. is seeking a Remote LTSS Service Care Manager in Pennsylvania to develop, assess, and coordinate holistic care management for long-term care members. You will educate members and families on services and benefits, and collaborate with care teams. The role requires a Bachelor’s degree and 2–4 years of related experience, with remote work and roughly 50% travel to nursing facilities, preferably near Lawrence, Mercer, Butler or Beaver counties.
