Remote Complex Care Manager, RN | LTSS Navigator
Molina Healthcare
Molina Healthcare is seeking a licensed RN to serve as a Care Manager for complex/intensive cases in Texas. The role is remote with 8 AM–5 PM CST hours, Monday through Friday, performing assessments and coordinating member care from home.
Responsibilities include comprehensive assessments, care planning, waiver processes, and collaboration with a multidisciplinary team to ensure high-quality, cost-effective care. Travel may be required 25–40% depending on state contracts.
