Remote RN LTSS Care Manager - Waiver & Coordination
Molina Healthcare
Molina Healthcare seeks an RN for a remote field-based care management role serving the greater Cook County area. You will conduct comprehensive assessments, coordinate waiver enrollment, and develop care plans with members, caregivers, and providers.
Regular travel is expected to support LTSS coordination and ensure high-quality, cost-effective member care. The role emphasizes risk assessment, MTSS integration, and collaboration with the ICT to authorize services and monitor outcomes.
