Remote RN Care Manager - LTSS Specialist

Molina Healthcare is seeking a Field-Based RN Care Manager with travel across the greater Cook County area. You will support LTSS care coordination, develop and monitor care plans, and collaborate with multidisciplinary teams to ensure high-quality, cost-effective member care.
