LTSS Care Manager — Remote with Local Travel

Molina Healthcare of Illinois

Molina Healthcare is seeking a Care Coordinator in Wisconsin to support care management and LTSS activities. The role involves collaborating with a multidisciplinary team to coordinate member care across settings and ensure progress toward outcomes.
