RN Care Manager — Remote Travel (WI)

Molina Healthcare

Molina Healthcare is seeking a registered nurse care manager to provide LTSS-focused care coordination, completing member assessments, developing care plans, and coordinating waiver services with a multidisciplinary team.

The role requires 25-40% local travel in Dodge, Fond du Lac and Green Lake Counties, home visits, ongoing monitoring of care plans, and collaboration with physicians, caregivers and other health professionals to improve member outcomes.
