Ambulatory Clinical Pharmacist, Infectious Disease

Job Summary In this ambulatory pharmacy role, the successful candidate will provide direct patient care within an infectious disease clinic while collaborating with a broad array of healthcare providers. Responsibilities include prescription processing, medication counseling, immunizations, and prior authorization management. Job Details Location: West Reading, Pennsylvania – onsite Hours: Monday to Friday, 7:00am – 5:00pm (no weekend or holiday shifts) Start date: To be determined Responsibilities Demonstrate the value of ambulatory pharmacy partnership to physician practices Maintain presence in clinic and handle medication questions from providers (formulary selection, dosing, ADRs, etc.) Ensure prescriptions flow through ambulatory pharmacy and are properly identified by pharmacy staff Maintain patient schedules for clinical pharmacist services (medication counseling, pillbox refills, immunizations, medication administration) Maintain patient lists and schedule appropriate follow‑ups to assess medication safety and efficacy, and communicate treatment plan recommendations with providers Coordinate patient refills and ensure auditable proof‑of‑delivery records are maintained Prepare and submit prior authorizations, manage denials and appeals Triage prescriptions that are limited distribution drugs or out‑of‑network Act as a financial advocate for patients when needed, leveraging charitable foundations, state programs, and manufacturer assistance to ensure medication affordability Navigate various platforms and document appropriately in EMR and specialty therapy management systems Contribute to the expansion of ambulatory pharmacy services through active participation in navigator committees and related projects Cross‑train in ambulatory pharmacy and assist with staffing as needed Education PharmD degree strongly preferred Current unrestricted pharmacist license in the appropriate state of practice 5+ years of community, retail, ambulatory, or health‑system pharmacy experience Experience in a specialty pharmacy and/or prior authorization processing is highly desirable Skills Proficient in Microsoft Office (Word, Excel, PowerPoint) Prior experience with pharmacy dispensing systems and insurance adjudication processes Comfortable working with clinical data, laboratory values, and patient medical records Benefits Comprehensive medical, dental, and vision plans Backup dependent care, adoption assistance, infertility coverage, and family building support Behavioral health solutions, paid parental and caregiver leave Professional development resources, mentorship programs, and employee resource groups Volunteer activity opportunities and employee wellness initiatives Equal Employment Opportunity Cencora is committed to providing equal employment opportunities without regard to race, color, religion, sex, sexual orientation, gender identity, genetic information, national origin, age, disability, veteran status, or any other protected class.
