This position offers flexibility, independence, and the opportunity to build meaningful relationships with patients while helping them improve mobility, safety, and quality of life in their own homes. Responsibilities: Provide skilled therapy services in patient homes. Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention. Monitor and communicate patient progress and changes to supervising PT. Encourage patient participation and reinforce goals. Complete timely and accurate documentation.
