ACT Psychiatric Nurse Practitioner
The ACT Psychiatric Nurse Practitioner functions as the psychiatric medical provider and senior prescribing clinician on the ACT team, directing, coordinating and providing medical/clinical services to members served by the team. Provides medication services, provides clinical leadership and guidance regarding team's delivery of direct clinical services, monitors member status, and participates in meetings, staffings, and treatment planning. Has a defined caseload, productivity goals, and meets the psychiatric needs of any member assigned to the ACT team. Provides after-hours care, on-call coverage, and support to all members served by the ACT team. Participates in utilization management and quality improvement activities as required.
Comprehensive psychiatric assessments, diagnostic clarification, and treatment planning
Ongoing psychopharmacologic management, including clozapine and long-acting injectable antipsychotics
Ability to integrate brief therapy during your visits as appropriate, to maximize clinical outcomes
Consultation with colleagues on complex or treatment-resistant cases
Providing psychiatric care for members on Court Ordered Treatment (COT) as part of your overall caseload
Coordination with primary care, pharmacy, and inpatient teams during care transitions
Work collaboratively with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, behavioral, medical and dental health care providers, community resource providers, and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.
Conduct frequent community visits, including but not limited to private homes, Southwest Network sites, provider/treatment/service locations, educational facilities, jail/juvenile facilities, hospitals, and residential treatment/group homes.

Outpatient only; no inpatient coverage
Serves as on-call as directed; appoints a designee when not available. Evening or weekend hours required on an as-needed basis
Manageable caseloads designed to prevent burnout and allow you to practice the way you trained to
Board-certified or eligible PMHNP
Experienced in working with SMI populations, ideally in community mental health or comparable settings
Psychologically-minded: skilled at both medication management and integrating brief therapy when appropriate, to care for the whole person
Comfortable navigating complexity and energized by the opportunity to make a difference in community mental health
A team player who values collaboration
Active Arizona PMHNP license
Valid DEA certification
Minimum two (2) years of experience as a psychiatric nurse practitioner, including experience treating adults with serious mental illness (SMI) and/or other high-acuity behavioral health populations. Prior ACT experience or equivalent community-based SMI experience strongly preferred.
Must maintain current CPR, First Aid, and Therapeutic Options (de-escalation/workplace safety) certifications, offered through Southwest Network
Valid Arizona driver's license and vehicle insurance
