About Our Nurse PractitionerPosition: The Psychiatric Nurse Practitioner position entailsdiagnosis, conducting assessments, and prescribing medications for patientsacross the lifespan, including children and adolescents, havingpsychiatric-mental health conditions, including those with serious mental illness(SMI). The role also includes developing an appropriate, integrated healthcaretreatment plan and providing effective patient care management with properevaluation and follow-up care. This position will provide telemedicineand in-person services based on patient and clinic needs. What a Nurse PractitionerDoes at Resilient Health: Plan, organize, and provide ongoingmedication management for new and existing patients Conduct initial psychiatric evaluationsperiodic medication management and monitoring visits Document patient visits in accordance withagency standards Review diagnostic test results anddocument the course of action taken Renew and refill medication to avoidlapses in treatment Provide emergency face-to-face counselingand/or crisis intervention as needed to avert, or in response to,patient relapse Develop and maintain effective workingrelationships with community agencies, case managers, primary careproviders, psychiatrists, and other members of the clinical team Participate in clinical staffing andindividual service planning sessions with case managers, treatment staff,family members, and other stakeholders to identify and meet patient'streatment needs Educate the patient in understanding thedisease process with guidance regarding preventative practices necessaryto manage acute and chronic conditions Order diagnostic tests and screeningevaluations Offer case consultation and clinicaltraining to Resilient team members Assist clients in identifying and pursuingeffective treatment goals Identify the types and amounts of supportclients require to remain independent despiteacute and chronicdiseases Maintain information on communityresources for clients' assistance and support, with support from teammembers Provide education, assistance, and supportto family members and others involved with the client to enhance awarenessof mental and emotional illness and to increase their ability tocontribute to the rehabilitation process. Requirements for ThisPosition: Advanced Practice Nurse PractitionerCertification from AZ State Board of Nursing as a Psychiatric/MentalHealth Nurse Practitioner (across the lifespan) required Master's in PMHNP Experience in a behavioral health settingpreferred Valid driver's license Valid CPR/First Aid certifications orability to obtain within 30 days of hire Valid Fingerprint Clearance Card orability to obtain within 90 days of hire Three-year Motor Vehicle Record that meetsour auto insurance carrier's coverage requirements Where You Will Be Working: 2255 W.
