Overview Under the supervision of the Chief Integration Officer for and within federal, state, and local guidelines, performs advanced and/or supervisory case management work. Work involves overseeing case management and program activities; developing program goals, objectives, and procedures; and evaluating outcome measures for the program. May plan, assign, and/or supervise the work of others. This position provides oversight of the Pre-Admission Screening and Resident Review (PASRR) and Enhanced Community Coordination (ECC) program. This position provides backup and assistance to their staff and other Local IDD Authority programs as needed. Ensures compliance with all applicable Center policies and guidelines, the Texas Administrative Code, HHSC Performance Contract, and applicable HHSC Handbooks. Adheres specifically to TAC codes related to Mental Illness and Intellectual and Developmental Disability PASRR Services. Will develop and monitor systems and procedures for maintaining high standards and consistency of work performed by staff. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgment. Education, Training, and Experience Graduation from an accredited four-year college or university with a degree in Human Services field to qualify as a QMHP-CS (Qualified Mental Health Professional-Community Services) At least one year of direct experience with Individuals with Intellectual and Developmental Disabilities to qualify as a QIDP (Qualified Intellectual Disability Professional). At least one year of case management or service coordination experience and two years of supervisory experience is preferred. Registration, Certification, Licensure, and Other Qualifications Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC). Must possess and maintain a valid driver’s license and automobile insurance. Individuals with an out-of-state driver’s license must be able to obtain a driver’s license in the state of Texas within thirty (30) days. Successful completion of all position-specific training within thirty (30) days of employment is required. Knowledge, Skills, and Abilities Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures. Ability to assess clients’ needs; to coordinate clients’ services; to monitor program effectiveness; to communicate effectively; and to plan, assign, and/or supervise the work of others. Ability to display regular and reliable (in-person, if required) attendance. Understanding and practice of Trauma Informed Care principles. The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed. Major Responsibilities Directly supervise the PASRR Habilitation Coordinators and Enhanced Community Coordinator by providing timely and required Habilitation Coordination training, ongoing supervision and documentation review. Completes required training as required by HHSC within the employee’s first 90 days of employment. Be available and supportive to staff. Review documentation as needed. Monitors delivery of Habilitation Coordination services to ensure compliance with all applicable Center policies and procedures and applicable Texas Administrative Codes, including 40 TAC, Chapter 9, Subchapter D and Chapter 2, Subchapter L. Develops, implements, and monitors systems and procedures for maintaining high standards, uniformity and consistency in the quality of work performed by staff. Ongoing review of client’s medical records charts, progress notes, and ISP’s to ensure uniformity, consistency, and high-quality work is being completed. Monitors all Service Coordination duties including, but not limited to: Completion of Habilitation Service Plans, PASRR Evaluations, and Individualized Service Plans. Monitoring of individual personal outcomes and service delivery. Accurate and timely documentation of Habilitation Coordination activity. Protection of Consumer Rights Completion of Permanency Plans Maintenance of documentation such as the current Habilitation Service Plan and Community Living Options Overview. Performs data entry in CARE, TMHP, and PUTTY. Monitors program specific duties and activities. Completion of Habilitation Service Plans, PASRR Evaluations, and Individualized Service Plans. Monitoring of individual personal outcomes and service delivery. Accurate and timely documentation of Habilitation Coordination activity. Protection of Consumer Rights Completion of Permanency Plans Maintenance of documentation such as the current Habilitation Service Plan and Community Living Options Overview. o Performs data entry in CARE, TMHP, and PUTTY. Monitors program specific duties and activities. Act as interim Service Coordinator as needed. Carries a varied, temporary, caseload during program vacancies. Provide support and consultation to individuals, and/or family members as needed. Maintains awareness of risk management issues and takes appropriate action to resolve potential risks to individuals, families, and employees. Available for assistance as needed and requested. Follows up with client issues in a timely manner. Maintains excellent working relationships with individuals, families, advocates, other agencies and other departments within the Center. Available for assistance as needed and requested. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Able to problem solve independently and handle crisis situations when they arise. Ensures compliance with all applicable federal, state and local laws, regulations, including but not limited to Center policies/procedures and the HHSC LA contract and applicable Texas Administrative Codes including 40 TAC, Chapter 2, Subchapter L and Chapter 9, Subchapter D. Maintains knowledge of the most current TAC, Handbooks, Performance Contract, and center policies and procedures. Ensures that changes are relayed to the Service Coordinators. Communicates all program needs and risk related issues to the Chief Integration Officer. Keeps Chief Integration Officer abreast on issues/situations as they occur. Interprets and prepares specialized reports as necessary or assigned. Runs CARE reports. Prepares various reports monthly, quarterly, and/or as needed. Assists the Chief Integration Officer in the overall operations of the department, especially in their absence. Serves as liaison between the Center, the Waiver Providers and community agencies. Maintain good working relationships with center staff, waiver providers, and other community agencies. Attends resource and transition fairs. Does presentations and trainings in the community as requested. Delivers a biannual educational opportunities to individuals in PASRR services. Provides ongoing monitoring of staff performance including evaluations and feedback of work performance. Competes employee evaluations within the designated timeframes. Completes ongoing supervisions to monitor work performance. Completes Employee Correct Action Forms when necessary. Organizes and prioritizes workload while remaining flexible to accommodate emergencies. Is available to staff when emergencies arise. Prioritizes tasks and assigns extras duties as necessary to the department. Perform additional duties as deemed necessary by department supervisor within designated timeframes. Completes additional assignments as requested. Maintains knowledge of developments in the field of mental health and intellectual disability and related services. Attends trainings and seminars as available. Listens to HHSC webinars as scheduled. Build and maintain positive relationships with nursing facilities. Ongoing interaction with local nursing facilities regarding the PASRR program. At times will maintain a small caseload of individuals with complex needs that require intensive case management. Handles HCS Diversion Slot requests and enrollments. Handles clients in crisis with imminent needs. Serves as the PASRR Diversion Coordinator as required in Attachment G and the DADS PASRR Reporting Manual. Provides ongoing education to service coordinators and other LIDDA staff. Provides educational presentations to PASRR clients/LARS at least semiannually. Within 45–75 days after an individual’s admission to a nursing facility, review the individual’s admission to ensure that community living options, services, and supports that could provide an alternative to nursing facility placement have been explored. Submits quarterly reporting to HHSC. Checks the TMHP portal daily for PASRR Alerts. Assigns PASRR Evaluations for completion within the designated timeframe. Completes PASRR Evaluations as necessary. Oversight of the Enhanced Community Coordinator as required in Attachment G. Ensures on-going monitoring is being performed in accordance with Attachment G of the Performance Contract. Supervisory Tasks & Standards Positions are posted and filled in compliance with Center procedures and timelines. Supervisor will provide individual written supervision to the staff supervised by the position, as needed. Supervision will provide guidance for outstanding needs of staff and contractors. (i.e. Review documentation for accuracy and content or provide input) Supervisor will effectively communicate with staff the information that is needed for supervisors to effectively manage their teams and programs. Positions are posted and filled timely. Newly hired staff receive supervisions from month of hire through sixth month of employment. Any staff having difficulties in performing their duties are receiving adequate supervision. Performance Evaluations must be completed within required timeframes and provide accurate depiction on employee’s performance and outline goals for improvement if needed. Ensures employees comply with education and training requirements, 100% compliance. Ensures that employees are trained to perform their jobs and that various requirements of the position are communicated to the employee. Supervisor will address performance issues and ensure that the staff is operating within acceptable Center guidelines. All subordinates staff are current in all trainings.
