SRSMG is a large, physician-led, multi-specialty group that offers patients convenient, coordinated & integrated care. Group provides patients a full range of services across 29 different specialties. All clinics have a lead RN, triage RN, LVNs and 1:1 Provider/MA ratio plus operations team. Integrated EMR system allows for seamless communication among group PCP's & specialists. Group has 200+ primary care providers throughout the county; built in referral system.

Requirements/Preferred Experience:

• SRSMG is seeking an experienced Family Nurse Practitioner or Physician Assistant to join our Sharp Extended Care Department
• Minimum of 2 years of full-time NP/PA experience in an outpatient setting
• Experience in geriatrics, post-acute care, hospice, home-based care, or skilled nursing facility (SNF) settings strongly preferred
• Active, unrestricted California NP or PA license required
• Current board certification in applicable specialty required
• Must be comfortable working independently
• Excellent communication skills with the ability to lead goals-of-care and advanced care planning discussions with patients and families

Position Details

• 1.0FTE full-time salaried position with a Monday-Friday schedule and one hour for lunch
• 100% outpatient position providing float coverage across Sharp Extended Care and Care at Home services
• Sharp Extended Care: manage post-acute patients in skilled nursing facilities, including patient rounding, care coordination, and discharge management
• Care at Home: conduct approximately 5 to 6 home visits per day, delivering intermittent skilled medical care to homebound patients in their residences
• Lead advanced care planning conversations and collaborate closely with patients, families, and interdisciplinary care teams
• No weekends / no inpatient care

Sharp Rees-Stealy Medical Group is committed to pay equity and transparency. Compensation is determined by a structured pay scale based on years of experience and applicable certifications, with final placement within the range confirmed during the recruitment process. The annual base salary range is $154,500.00-$216,537.30, with eligibility for bonuses, incentives, and a comprehensive benefits package. This position is represented by UNAC/UHCP under applicable labor agreements.
