Home Health Admission Nurse (RN) — OASIS & Care Planning
Infinity Hospice Care LLC
Infinity Hospice Care LLC is seeking an Admission Nurse (RN) to coordinate and deliver comprehensive home health care. You will assess patients, plan and document care, complete OASIS assessments, and determine additional disciplines needed to support the patient’s needs. You will educate staff, participate in on-call rotations, and perform 3–4 OASIS visits daily within the assigned region, ensuring timely, compliant documentation and care planning. #J-18808-Ljbffr Infinity Hospice Care LLC
