Care Management Coordinator II - Member Care & Coordination

Care Health Plan in Los Angeles is seeking a Care Management Coordinator II to outreach to members, caregivers, providers, and community partners to enroll high-need members into CM and DM programs. You will assess members' strengths, challenges, and barriers to care via telephonic HRA and support care planning with the ICT. You will coordinate information flow between CM team, providers, medical groups, and ICT; reinforce health education, coordinate benefits such as transportation and #J-18808-Ljbffr Socket.dev
