Work Environment

This position is primarily onsite, providing home care services to patients in Yonkers and surrounding areas. As a Physical Therapist in our organization, you will work in diverse settings, visiting clients in their residences to deliver therapy in a comfortable and familiar environment. This arrangement allows you to cultivate deeper relationships with your patients, facilitating trust and open communication crucial for their rehabilitation progress. Your schedule will involve a consistent Monday to Friday commitment, with a total of 40 hours per week, allowing for work-life balance while serving our communities effectively.

Key Responsibilities

• Evaluate patients referred by physicians, developing comprehensive, individualized treatment plans based on clinical assessments and patient history.
• Implement evidence-based interventions tailored to meet the unique needs of patients, ensuring alignment with physician-directed care plans.
• Provide hands-on therapeutic care in patients' homes using state-of-the-art techniques and tools designed to facilitate optimal recovery and functional improvement.
• Encourage patient engagement through education, helping them understand their conditions and empowering them in their recovery process, which may also involve training caregivers to assist effectively.
• Document progress, maintaining accurate and up-to-date records of patient therapy encounters, treatment progress, and any changes in the patient's functional status.
• Collaborate with a multidisciplinary team to ensure comprehensive care and improve patient outcomes, participating in case reviews and team meetings regularly.
• Exceed business objectives by meeting goal metrics for patient utilization, satisfaction, and retention.

Patient Population / Client Focus

Our Physical Therapists work with a diverse range of patient populations, including orthopedic, pediatric, neurologic, and sports medicine cases. You will have the opportunity to tailor your approach to varied age groups and clinical conditions, providing therapy that is both thoughtful and effective. Qualifications

• A Bachelor's degree in Physical Therapy from a program accredited by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy Education (CAPTE).
• Current and valid state licensure to practice Physical Therapy in New York.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Strong verbal and written communication skills, capable of creating a supportive rapport with patients.
• Proficient in utilizing electronic documentation systems and word processing software.
• A demonstrated ability to establish and nurture therapeutic relationships with patients to boost their motivation and achieve functional goals.
• A genuine desire to connect positively and meaningfully with patients and their families.

Preferred Experience

• Prior experience in home health or outpatient physical therapy settings is a plus.

Compensation and Benefits

We offer a competitive yearly salary ranging from USD 100,000 to 120,000, based on experience and qualifications, along with a robust benefits package designed to support our employees' well-being.
