Physical Therapist ~ PT

Stellar Senior Living is seeking a dedicated and compassionate Physical Therapist (PT) to join our Healthcare & Medical Services team, supporting residents in our vibrant senior living community. In this role, you will provide comprehensive, evidence-based physical therapy services designed to enhance mobility, independence, safety, and overall quality of life for older adults in independent living, assisted living, and memory care settings.

As a Physical Therapist with Stellar Senior Living, you will complete thorough assessments of residents’ functional abilities, including strength, range of motion, balance, gait, pain levels, and fall risk. You will design and implement individualized treatment plans focused on functional goals such as safe transfers, ambulation, fall prevention, and improved participation in daily activities. Interventions may include therapeutic exercise, neuromuscular re-education, balance and gait training, functional mobility training, and use of modalities as appropriate and permitted by policy and state regulations.

You will work in close collaboration with nursing staff, physicians, occupational therapists, and other members of the interdisciplinary team to coordinate care and ensure that treatment plans align with each resident’s medical status and personal goals. Regular communication with residents and their families is essential, as you educate them on home exercise programs, safe mobility strategies, proper use of assistive devices, and approaches to maintaining gains achieved in therapy.

Accurate and timely documentation is a core aspect of the position. You will complete evaluations, daily treatment notes, progress reports, and discharge summaries that meet clinical, regulatory, and organizational standards. You will track functional outcomes and adjust treatment plans based on progress, changes in condition, or new clinical information.

You should be comfortable working with geriatric populations, including residents with mobility limitations, chronic conditions, and cognitive impairments such as dementia.
