Physical Therapist ~ PT

Mantey Heights Rehabilitation & Care Center, part of the Stellar Living family in Grand Junction, CO, is seeking a licensed Physical Therapist (PT) to join our Healthcare & Medical Services team. In this role, you will provide high-quality, resident-centered physical therapy services to individuals in our assisted living, memory care, and independent living settings. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to help residents restore function, improve mobility, reduce pain, and maintain their highest possible level of independence. Working closely with an interdisciplinary care team—including nurses, caregivers, physicians, and family members—you will communicate therapy goals, progress, and discharge plans, ensuring a coordinated approach focused on wellness, safety, and quality of life. Daily responsibilities include conducting assessments, documenting in the electronic medical record in accordance with regulatory and facility standards, educating residents and families on home exercise programs and safe mobility techniques, and monitoring response to treatment, adjusting plans as needed. You will also provide caregiver training on proper body mechanics, transfers, and fall-prevention strategies to support resident comfort and happiness. The ideal candidate is compassionate, resident-focused, and committed to evidence-based practice, with strong clinical judgment and time-management skills. You should be comfortable working with older adults, including those with dementia and complex medical conditions, and be able to motivate and encourage residents with patience and respect.
